[Phrf] Ratings Forms

Good day all,

As you probably know by now the club will be announcing some form of racing
sometime next week or weekend. If you want to be scored and have not sent
in your form or corrected form due to issues, then you need to do it ASAP.
Please follow the process on the website. We do not want to have to make a
bunch of corrections when forms are trickling in.
