[Phrf] Forms & Check Process

The process for submitting your ratings form(s) and paying your dues has
not changed and is documented on the website. The process is not perfect
but it is what we have. Please follow that documented process as it makes
sure the forms are submitted and dues are paid accordingly and we know who
has done so.
